As this damage gets worse, your kidneys stop working as well and protein can leak into your urine. In some cases, IgAN can lead to kidney failure. Trutakna blocks two signals in the immune system called BAFF and APRIL. These signals help certain immune cells, called B cells, grow and survive. By blocking both signals, Trutakna may help lower the levels of abnormal IgA in your body. This could help reduce the amount of damage to your kidneys over time.

Why Was It Approved?

The FDA approved Trutakna based on the results of an ongoing study in about 200 adults with IgAN. In this study, the medicine was compared with a placebo (a treatment with no medicine in it). Before starting the study, everyone was already taking oral medicines to help protect their kidneys. These other medicines were continued throughout the study.

People who used Trutakna every week for about nine months had lower levels of protein in their urine than people who received the placebo. On average, people using the medicine had about a 46% drop in urine protein from where they started. This was significantly better than the change seen in the placebo group (about 7%). A drop in urine protein is expected to predict a real benefit for patients, but this is not yet proven. To confirm whether Trutakna can help the kidneys work better for longer, this study is still running. At a later time, researchers will measure the kidney function of everyone in the study. This will provide helpful information on whether this medicine can help slow the loss of kidney function over time. Side effects were common, but most were mild or moderate. The most common side effects were infections, including upper respiratory infections like a cold. The next most common side effects were reactions where the shot was given, such as redness.

What Do I Need to Know?

Trutakna is given as a shot under your skin once a week. It comes in an autoinjector, which is a device you can use at home. The shot is given in your thigh or belly by either you or a caregiver. Before you start this medicine, talk to your health care provider about all medicines, vitamins, minerals, and supplements that you take. Your health care provider may have you continue other medicines that help protect your kidneys. But you may need to stop certain medicines that can affect your immune system. Trutakna can weaken your body’s defenses against infections. Before you start treatment, talk with your health care provider about whether you need any vaccines. Your health care provider should also check that you do not already have an infection. While you are taking Trutakna, watch for signs like a fever, pain when you urinate, or coughing. Call your health care provider right away if you notice these signs. If you get a serious infection, you may need to stop treatment until you feel better. It isn’t clear whether Trutakna is safe to use while pregnant or breastfeeding. If you are pregnant or trying to get pregnant, speak with your health care provider about the potential risks and benefits before starting treatment.

Trutakna (atacicept) is an injection that is approved for adults with a kidney disease called primary immunoglobulin A nephropathy, or IgAN.
