It is now approved to lower the risk of major heart problems, including heart attacks, strokes, and death from heart disease, in adults with type 2 diabetes who have a high risk of these events. Mounjaro was already approved to manage blood sugar levels alongside diet and exercise in adults and children 10 years and older with type 2 diabetes.

Mounjaro works by targeting two natural gut hormones in the body called GIP and GLP-1. These hormones help the body release insulin when blood sugar levels rise, lower glucagon levels, and slow down digestion. This combination helps lower fasting and post-meal blood sugar levels and can decrease appetite.

Why Was It Approved?

The new approval was based on a large clinical trial named SURPASS-CVOT that included nearly 13,300 adults with type 2 diabetes and established heart disease. Over a median follow-up time of about four years, researchers compared Mounjaro directly against Trulicity (dulaglutide) a diabetes injection proven to protect heart health. The main goal was to evaluate whether Mounjaro reduced the combined risk of heart attack, stroke, or heart-related death as effectively as Trulicity.

The study showed that Mounjaro was non-inferior to Trulicity, with patients taking Mounjaro experiencing an 8% lower overall rate of major heart events compared to those taking Trulicity. Specifically, 12.1% of patients on Mounjaro had a major heart event compared to 13% of patients taking Trulicity. Mounjaro did not demonstrate superiority over Trulicity. Overall, the trial showed that Mounjaro provides significant heart protection while helping patients manage their diabetes.

What Do I Need to Know?

Mounjaro is given as a subcutaneous (under the skin) injection once a week into your stomach, thigh, or upper arm. You should rotate your injection site every week.

The most common side effects of Mounjaro affect the digestive system and include nausea, diarrhea, decreased appetite, vomiting, constipation, indigestion, and stomach pain. These issues are usually mild to moderate and occur most often while your healthcare provider is slowly raising your dose.

Mounjaro carries a warning because it causes thyroid tumors in rats, though it is unknown if it causes thyroid cancer in humans. You should not take Mounjaro if you or anyone in your family has ever had medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2. Serious risks include pancreatitis, acute gallbladder disease, low blood sugar when used with insulin, dehydration that may cause kidney injury, and severe allergic reactions. Because Mounjaro slows stomach emptying, you must tell your healthcare provider about any planned surgeries involving anesthesia. If you take oral birth control pills, Mounjaro may reduce their effectiveness. You should switch to a non-oral birth control method or add a barrier method (like condoms) for four weeks after starting Mounjaro and for four weeks after each dose increase. Talk to your healthcare provider if you are pregnant or planning to become pregnant, as Mounjaro may cause harm to a fetus.

What Is Mounjaro and Why Does It Matter?
