Before this approval, this medicine was already available for adults with a rare blood cancer called polycythemia vera.

Essential thrombocythemia is a condition that affects the bone marrow, which is the spongy tissue inside bones where blood cells are made. In people with this condition, the bone marrow makes too many platelets. These are small blood cells that help stop bleeding. Having too many platelets can lead to dangerous blood clots or sudden bleeding problems.

Besremi belongs to a group of medicines called interferons. It works by attaching to special target areas in the bone marrow to control cell production. This helps lower the number of platelets in the blood.

Why Was It Approved?

The FDA approved Besremi after reviewing a clinical study of 174 adults with essential thrombocythemia. The people in this study had already tried taking another drug, called hydroxyurea, but either did not get better while taking it or could not tolerate it. In the study, 91 patients received Besremi shots every two weeks, and 83 patients took a different medicine, called anagrelide, by mouth. Everyone also took low-dose aspirin.

Researchers looked at how many patients reached healthy platelet counts, had no blood clots or bleeding, and did not have any growth in their spleen. After nine and 12 months, 37% of patients taking Besremi reached this goal, compared to only about 4% of patients taking anagrelide. The most common side effects in the study were higher liver enzymes, low red blood cell counts, fevers, bacterial infections, itching, and weight loss. A small number of patients using Besremi had serious side effects.

What Do I Need to Know About Besremi?

Besremi is given as a shot under the skin of your stomach or thigh. It is injected once every two weeks. A healthcare provider can train you or your caregiver to give the shot using an injection pen or syringe.

You should not take Besremi if you have a history of severe depression, serious liver problems, or serious autoimmune disease. Besremi carries a severe warning for mental health issues, such as deep depression or thoughts of suicide. Tell your healthcare provider right away if you notice changes in your mood.

Common side effects of the medicine include liver damage, fatigue, hair loss, and diarrhea. Tell your health care provider about all of your medical conditions, including diabetes, a thyroid problem, heart disease, bleeding or clotting problems, or kidney or liver issues.Your healthcare provider will check your blood counts and examine your eyes during treatment. Brush your teeth often and get regular dental checkups to protect your gums. Do not drive or use machinery if Besremi makes you feel dizzy or sleepy. Always tell your healthcare provider about all medicines you take.

Besremi can harm a fetus. Females who can get pregnant must get a pregnancy test before starting treatment. They should use effective birth control during treatment and for eight weeks after their final shot. Do not breastfeed while using Besremi and for eight weeks after your last dose.

What Is Besremi, and Why Does It Matter?
